Job Summary

Vine Medical Group is seeking a skilled Nurse Practitioner to join our Same Day Care Team. As an Advanced Nurse Practitioner, you will provide high-quality clinical care to patients with undifferentiated physical and mental health conditions.

Main Responsibilities
  • Deliver timely and effective clinical care to patients, assessing their needs and providing holistic treatment.
  • Act as a first point of contact for patients, offering assessment and referral to specialist services when necessary.
  • Work autonomously within your scope of practice, accessing the wider clinical team when required.
  • Supply, administer, and prescribe medicines as indicated, adhering to medicines management policy and NMC/HCPC standards.
  • Provide telephone and video consultations, including safe and effective triage of acutely unwell patients.
  • Conduct reactive home visits to assess, plan, treat, and arrange admission to hospital if required.
  • Offer advice on general health care, lifestyle, and referrals to internal or external services.
  • Make appropriate referrals to primary, secondary, and social care services, following practice pathways and protocols.
Person Specification
  • Clinical examination skills across a diverse range of patients, including children and adults.
  • Able to build positive rapport with patients and colleagues, communicating complex information effectively.
  • Working knowledge of EMIS and Docman medical systems.
  • Registered Nurse Practitioner/Advanced Nurse Practitioner with NMC, qualified to Master's degree or equivalent, and full UK driving license.
  • Accredited training in urgent care, chronic disease management, or frailty, and non-medical prescriber.
Requirements
  • 3+ years of experience as a Nurse Practitioner/Advanced Nurse Practitioner.
  • Background in fast-paced Primary Health Care or acute environment.
  • Proven record of acting autonomously and responsibly for patient assessment, treatment, and referral.
