About the Role

We are seeking a dedicated and compassionate individual to provide high-quality patient care and support to those at the end of their life. As a Palliative Nursing Specialist, you will be responsible for assessing and developing individualized care plans, working collaboratively with the clinical team to ensure seamless care delivery.

What You'll Do
  • Deliver expert palliative nursing care, informed by evidence-based practice, and aligned with patient wishes.
  • Support patients and their families in open and honest conversations about end-of-life care, fostering a sense of control and empowerment.
  • Collaborate with the multidisciplinary team to develop and implement effective intervention and support plans, ensuring patient-centered care.
  • Contribute to the supervision and education of other healthcare professionals, promoting a culture of learning and development.
  • Participate in evidence-based projects and audits, driving continuous improvement in care quality and outcomes.
