Sanctuary Personnel Ltd is seeking a skilled Social Worker to join our Adult Learning Disability Team in Bromley. As a key member of our team, you will work closely with vulnerable adults and their carers to ensure their wellbeing and safety.
Key Responsibilities- Adhere to the ethos of wellbeing when working with vulnerable adults and their carers.
- Participate in duty operations and respond to safeguarding enquiries and Best Interest Assessments as required.
- Provide assessment services to individuals, their carers, and other agencies in accordance with Care Act 2014 criteria.
- Initiate and coordinate care and support plans for individual service users and carers, in conjunction with professional colleagues and representatives of voluntary and private agencies.
- Monitor the efficiency and effectiveness of individual care and support plans.
- Review the needs of individual service users and carers, and maintain client records in accordance with Council Policy.
- Respond to and resolve complaints within the Council's complaints procedure.
- Respond to new and changed legislation in respect of Adult Services and implement as directed.
- Ensure effective use of all available resources, including the voluntary sector, to meet the eligible needs of adults.
- Coordinate, participate in, and chair case conferences and planning meetings, including safeguarding meetings.
