We are seeking a highly skilled and experienced Community Care Officer (OT) to join our team at Nottinghamshire County Council. As a key member of our Maximising Independence Service (MIS), you will play a vital role in promoting independence and wellbeing for individuals in the Mid Notts area.

Key Responsibilities

  • Deliver individually tailored reablement programmes to prevent and reduce ongoing care and support needs.

  • Work closely with the reablement team to ensure seamless delivery of services and effective outcomes.
  • Develop and maintain strong relationships with partner agencies and stakeholders to ensure effective collaboration and communication.

Requirements

  • Good understanding of how psychological, cognitive, and medical conditions impact upon individuals' abilities to function independently.
  • Experience of carrying out functional assessments, goal planning, and equipment/adaptation prescription.
  • Excellent clinical, peer, and management support is provided with extensive training and development opportunities.
  • Advanced remote working technology is provided to maximise efficiency, minimise travel, and support work/life balance.
