Our team in Cumbria, Northumberland, Tyne and Wear (CNTW) NHS Foundation Trust is seeking a highly skilled Community Practitioner to join our Early Intervention in Psychosis team.

About the Role

This is an exciting opportunity for a qualified practitioner to work with individuals experiencing their first episode of psychosis. As a Community Practitioner, you will be part of a dynamic team that provides holistic assessments and delivers evidence-based therapeutic interventions to support recovery and mental health needs.

The successful candidate will work closely with service users, families, and carers to provide care coordination and support on a recovery-focused journey. You will also have opportunities to develop your skills and knowledge through ongoing training and professional development.

Key Responsibilities
  • To work as a member of the Multi-disciplinary Team ensuring high-quality individualized care delivery.
  • To carry out comprehensive risk assessments and devise risk management plans for clients using the Biopsychosocial Risk Assessment Tool.
  • To deliver evidenced-based clinical and psychologically based interventions.
  • To participate in duty rota and triaging new referrals.
  • To support health promotion of service users and promote a culture of learning, education, and professional development.
Person Specification

We are looking for a candidate who has:

  • A Professional Qualification and current Professional Registration as a First-Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker, or Clinical Psychologist.
  • Evidence of post-registration training in clinical/psychological interventions/skills relevant to the post.
  • Proven track record of post-registration clinical experience relevant to the post.
  • Mentor Training.
  • Training in CPA (Care Programme Approach), Safeguarding, MAPPA (Multi Agency Panel for the Protection of Vulnerable Adults), and Child Protection.
  • Up-to-date knowledge/experience of relevant legislation.
What We Offer

We offer a competitive salary of £35,000 - £40,000 per annum, depending on experience, plus benefits including pension scheme, annual leave, and opportunities for career progression.
