About the RoleThis is an exciting opportunity for a qualified practitioner to work with individuals experiencing their first episode of psychosis. As a Community Practitioner, you will be part of a dynamic team that provides holistic assessments and delivers evidence-based therapeutic interventions to support recovery and mental health needs.
The successful candidate will work closely with service users, families, and carers to provide care coordination and support on a recovery-focused journey. You will also have opportunities to develop your skills and knowledge through ongoing training and professional development.
Key Responsibilities- To work as a member of the Multi-disciplinary Team ensuring high-quality individualized care delivery.
- To carry out comprehensive risk assessments and devise risk management plans for clients using the Biopsychosocial Risk Assessment Tool.
- To deliver evidenced-based clinical and psychologically based interventions.
- To participate in duty rota and triaging new referrals.
- To support health promotion of service users and promote a culture of learning, education, and professional development.
We are looking for a candidate who has:
- A Professional Qualification and current Professional Registration as a First-Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker, or Clinical Psychologist.
- Evidence of post-registration training in clinical/psychological interventions/skills relevant to the post.
- Proven track record of post-registration clinical experience relevant to the post.
- Mentor Training.
- Training in CPA (Care Programme Approach), Safeguarding, MAPPA (Multi Agency Panel for the Protection of Vulnerable Adults), and Child Protection.
- Up-to-date knowledge/experience of relevant legislation.
We offer a competitive salary of £35,000 - £40,000 per annum, depending on experience, plus benefits including pension scheme, annual leave, and opportunities for career progression.
