Nurse Opportunity at The Fremantle Trust

We are seeking a skilled and compassionate Nurse to join our team at Lewin House, a modern and spacious care home in Aylesbury. As a Nurse, you will play a vital role in providing high-quality nursing and social care to our residents, ensuring their comfort, dignity, and well-being.

Key Responsibilities:
  • Provide respectful and person-centred care to residents, acting as a role model for excellent practice.
  • Support the staff team to achieve excellent nursing and social care standards.
  • Plan, write, implement, and review Support Plans with input from residents, relatives, and key members of the care team.
  • Ensure residents receive person-centred care that meets their assessed clinical and social care needs.
  • Assist residents with tasks of daily living, such as communication, social engagement, personal care, mobility, and end-of-life care.
  • Support residents to take their medication as prescribed and recorded in the Medication Administration Record.
  • Ensure residents have companionship and social, emotional, and leisure support in accordance with their wishes and Support Plans.
  • Record essential information in appropriate locations clearly, accurately, and immediately.
Requirements:
  • A valid NMC Pin.
  • Evidence of clinical training.
  • A minimum of 2 years previous experience working within a nursing home.
  • Experience of care planning and key working principles.
  • Understanding of relevant health & safety legislation.
  • Working knowledge of the Regulatory Framework relating to the care of Older People.
