Key Responsibilities:
- Assess and monitor individuals on a one-to-one basis, providing support and guidance to help them reach their goals.
- Work collaboratively with a variety of organizations within the locality to provide comprehensive support services.
- Provide emotional support, face-to-face, online, or via telephone, using active listening, de-escalation, and distraction techniques to prevent admission to emergency departments.
- Work flexible shifts, including evenings and weekends, to meet the needs of our service users.
