Job OverviewWe are seeking an experienced Operational Manager to lead our innovative service, which includes urgent community response, falls pick-up services, seven-day social care, and virtual ward.
This is an integrated role working across both health and social care, jointly funded by the Royal Borough of Greenwich and Oxleas NHS Foundation Trust. The post holder will also be responsible for managing the Royal Greenwich Adult Social Care Emergency Duty Team.
About the RoleThe successful candidate will lead a multi-professional team providing a rapid response within 2 hours to proactively prevent hospital admission and meet the needs of patients in the community. The team provides support, treatment, and monitoring on our virtual ward for up to fourteen days and uses digital healthcare monitoring.
The post holder will also be responsible for an on-call rota, delivering and managing the Royal Greenwich Adult Social Care Emergency Duty Team, which operates from 17:00.
