Job DescriptionThis is an exciting opportunity for a skilled and experienced Care Manager to join our team. As a Care Manager for Supported Living Services, you will be responsible for overseeing the provision of care and support to individuals with learning disabilities.
You will work closely with our team to ensure that the needs of our service users are met, and that they receive the highest quality of care and support.
Your key responsibilities will include:
- Managing a team of staff to provide care and support to individuals with learning disabilities;
- Developing and implementing care plans to meet the needs of our service users;
- Monitoring and evaluating the effectiveness of our services;
- Working closely with other agencies to ensure that our service users receive a comprehensive range of support.
We are looking for someone who has a strong background in care management, and who is able to lead and manage a team effectively.
Required Skills and QualificationsTo be successful in this role, you will need:
- A relevant qualification in care management, such as NVQ/QCF Level 3 in Health & Social Care / Management;
- Experience of working in care management, preferably in a supported living setting;
- Strong leadership and management skills;
- Excellent communication and interpersonal skills;
- Ability to work independently and as part of a team.
