Responsibilities:
- Provide flexible support and advice to young people and adults preparing to leave care.
- Work closely with service users to ensure the best possible outcomes for them.
- Support other social workers in the team.
Requirements:
- A degree in Social Work and registered with Social Work England.
- An up-to-date DBS check.
- Experience working in Leaving Care/Children in Care or transitions.
