Main Responsibilities:

As a Salaried General Practitioner, you will be responsible for providing high-quality patient care in a fast-paced and dynamic environment. Your duties will include:

  1. Providing face-to-face and telephone consultations for routine and urgent care.
  2. Conducting patient home visits (low frequency).
  3. Managing patient care in surgery, including investigating and reviewing results, developing care plans, and making referrals to secondary care or other providers.
  4. Issuing and signing repeat prescriptions, ensuring they align with national/local guidelines.
  5. Administering and handling patient-related queries, test results, referral letters, and other tasks in a timely manner.
  6. Recording clear, contemporaneous consultation notes using EMIS and S1 clinical systems.
  7. Ensuring safe, evidence-based, and cost-effective care, in line with Practice disease management protocols and NICE guidance.
  8. Independently prescribing medications within your scope of practice.
  9. Reviewing and managing patient medications following appropriate policies and local clinical guidelines.
  10. Utilising other available clinical and support staff in delivering holistic and patient-centred care.
  11. Actively participating in MDT and care conference meetings, as required.
  12. Participating in on-call duties on a pro-rata basis, if required.
Health Promotion and Patient Support:

You will also be responsible for:

  1. Regularly screening for lifestyle factors and supporting patients in adopting health promotion strategies and self-care principles.
  2. Contributing to the delivery of anticipatory care plans.
Professional Development:

To ensure you remain up-to-date with the latest clinical developments, you will:

  1. Maintain awareness of clinical practice developments.
  2. Attend weekly huddle meetings, Friday Lunchtime training, and clinical meetings.
  3. Participate in an annual performance review and take responsibility for personal development.
  4. Engage in mandatory training and attend courses as needed.
Team Collaboration:

You will work closely with:

  1. Providing clinical support and guidance to both clinical and administrative staff to maintain the highest care standards.
  2. Collaborating with health and social care providers to deliver an integrated, safe service.
Risk Management and Compliance:

You will be responsible for:

  1. Adhering to all practice policies and procedures, including those related to prescribing, confidentiality, and data protection.
  2. Applying infection control measures and managing risks in compliance with local and national guidelines.
  3. Contributing to clinical governance through audits, peer reviews, and significant event analysis.
Patient Safety and Confidentiality:

You will ensure:

  1. Patient and carer confidentiality, in line with practice policies.
  2. Managing patient data securely, only disclosing it to authorised persons.

  1. Respect patient confidentiality and manage sensitive information appropriately.
  2. Only disclose information to authorised persons in accordance with Practice policies.
Health & Safety:

You will:

  1. Assist in promoting and maintaining health, safety, and security as defined in the PCN Health & Safety Policy.
  2. Identify risks involved in work activities and undertake such activities in a way that manages those risks.
  3. Make effective use of training to update knowledge and skills.
  4. Use appropriate infection control procedures and maintain work areas in a tidy and safe way.
  5. Report potential risks identified.
Safeguarding:

You will:

  1. Promote and protect against any safeguarding concerns relating to Children and Adults.
  2. Recognise and report any concerns regarding Vulnerable Adult issues to the appropriate authorities.
  3. Recognise and report any concerns regarding Child Protection issues, including possible child abuse.
  4. Understand duties in relation to the Mental Capacity Act.
