We are seeking a skilled and compassionate Community Staff Nurse to join our team at Central and North West London NHS Foundation Trust. As a Community Staff Nurse, you will play a vital role in delivering high-quality nursing care to patients in their own homes.
Key Responsibilities- Provide comprehensive nursing care to patients with a range of needs, including wound care, medication management, and palliative care.
- Work autonomously and as part of a multidisciplinary team to deliver patient-centered care.
- Develop and maintain effective communication with patients, carers, and healthcare professionals to ensure seamless care coordination.
- Participate in regular supervision and training to enhance skills and knowledge.
- Contribute to the development of the district nursing service, identifying opportunities for improvement and implementing changes to enhance patient care.
- NMC registration as a Registered Nurse.
- Experience of working in a community setting or willingness to learn and adapt to this environment.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, carers, and healthcare professionals.
- Ability to work autonomously and as part of a team, with a strong focus on patient-centered care.
- Sound IT skills and ability to maintain accurate records.
