Job Overview

We are seeking a skilled Mental Health Practitioner to join our team in Primary Care, working as part of a General Practice team in SWIFT Primary Care Network practices in South Bristol. The successful candidate will work a Wednesday to Friday schedule.

This is an exciting opportunity to be part of a redesigned community mental health service, providing a personalized, preventative, and proactive integrated approach. Our team is working together with Primary Care, voluntary services, and Avon & Wiltshire Mental Health Partnership NHS Trust to provide a one-team approach for our local populations.
