About the RoleAs a Primary Care Nurse, you will work within a multi-disciplinary team to deliver a nurse-led service and prevent hospital admissions. Your responsibilities will include undertaking medication rounds, dispensing controlled medication, attending emergency response situations, assessing patients, and undertaking reception screening. You will also use your skills and knowledge to deliver nurse-led clinics to your patient group, while being supported by a multi-disciplinary team.

RequirementsTo be successful in this role, you will need:
- Registration as a Registered Nurse Adult with the Nursing and Midwifery Council (NMC).
- Current professional qualification (NMC).
- Evidence of Continued Professional Development.
- Understanding of the complexities of multi-disciplinary and multi-agency working.
- A non-judgmental and compassionate approach.
- Excellent communication skills.
- Understanding of clinical governance, health and safety, QOF, EBP, and national standards.
- Knowledge of healthcare policies and best practice.
