About the Role:We are seeking a highly skilled Primary Care Provider to join our team. As a key member of our care team, you will be responsible for providing comprehensive primary care services to patients in their homes, community settings, and through our state-of-the-art mobile clinic. Your expertise will be invaluable in helping us achieve our mission of transforming healthcare.
Key Responsibilities:- Conduct initial patient visits and establish relationships with patients and their families
- Provide primary care services, including annual wellness visits, establish-care visits, and transitional care
- Collaborate with a multidisciplinary team of experts to manage patient care and improve health outcomes
- Identify and address financial, social, environmental, or functional barriers to care or better health
- Participate in population health initiatives to drive improvements in our care model and reduce inefficiencies
We are looking for a highly motivated and compassionate Primary Care Provider with a strong passion for delivering high-quality patient care. You should have a minimum of 2 years of clinical experience in complex care, chronic care, and primary care. Your innovative leadership qualities, proven experience with successful behavior modification, and comfort with basic procedural skills will be essential in this role.
