Occupational Therapist

We are seeking an experienced Occupational Therapist to join our dynamic and innovative team in the Intermediate Care service. As an Occupational Therapist, you will be responsible for providing specialist community therapy assessment and treatment to patients in their own homes and residential care settings.
Key Responsibilities- Assess and manage a caseload of patients with complex and/or multiple needs, working closely with the multidisciplinary team to provide high-quality rehabilitation and support.
- Develop and implement individualized treatment plans to optimize patient outcomes and promote independence.
- Collaborate with other healthcare professionals to ensure seamless care coordination and effective communication.
- Contribute to the development and implementation of service policies and procedures, ensuring compliance with clinical governance and quality standards.
- Participate in education and training activities to maintain professional development and stay up-to-date with best practices.
- Degree or equivalent in Occupational Therapy.
- Current HPC Registration.
- Ability to demonstrate specialized clinical reasoning and treatment skills.
- Good level of clinical reasoning, handling, and treatment skills.
- Effective communication and interpersonal skills.
- Awareness of NHS/Clinical Governance issues.
- Ability to risk assess for own and others' work area and caseloads.
