Job Title: Social Care Professional

We are seeking a highly motivated and experienced Social Care Professional to join our team at Oldham Council. As a Social Care Professional, you will play a vital role in providing assessments of need, care and support planning, and undertaking safeguarding work to ensure the well-being of adults of working age and/or older people.

Key Responsibilities:
  • Manage complex cases in accordance with statutory responsibilities and national and local policies and procedures.
  • Carry out assessments and planning of individual packages of care for service users and carers, in line with criteria of eligibility and designated care plan.
  • Work in a Strengths-Based and holistic, person-centred way, ensuring customers and families remain central to the processes they are part of.
Requirements:
  • A good understanding of the legislative frameworks underpinning adult social care, including the Care Act and the Mental Capacity Act (MCA).
  • Excellent organisational skills to work in conjunction with, and provide consultation to partner agencies to deliver effective planning for Adults, their carer's and families to ensure positive outcomes.
