Key Responsibilities
  • Provide person-centered support to encourage customers to maximize their potential and independence
  • Assess customers' needs and develop tailored support plans
  • Act as a first point of contact for customer queries and concerns
  • Build strong relationships with customers, promoting personal choice, privacy, and dignity
  • Liaise with external agencies to enhance customers' confidence, health, and wellbeing
  • Respond to emergency situations and maintain accurate records
Requirements
  • Experience working with adults who have experienced homelessness
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Organizational skills and functional literacy and numeracy
  • NVQ3 in Health and Social Care or willingness to work towards
