About the Role

We are seeking a highly skilled Young Person Transition Support Manager to join our team at South West London and St George's Mental Health NHS Trust. As a Transition Support Manager, you will be responsible for managing the transition of young people from child and adolescent mental health services to adult services.

The successful candidate will have a strong background in clinical work, with experience of working with adolescents and their families. You will be responsible for leading on safeguarding issues, developing services, and providing supervision to other team members.

About the Salary

The estimated salary for this role is £50,000 - £65,000 per annum, depending on experience.

About the Location

This role will be based in Richmond, London, with easy access to public transport links.

Job Description

Key Responsibilities:

  • Lead on safeguarding issues for young people transitioning to adult services
  • Develop services to meet the needs of young people transitioning to adult services
  • Provide supervision to other team members
  • Collaborate with multi-agencies to ensure wrap-around support for young people
  • Develop innovative ways of meeting the needs of young people from diverse backgrounds
