About the Role

We are seeking a highly motivated and enthusiastic individual to join our Newcastle Adult Community Mental Health Team at North Staffordshire Combined Healthcare Trust. As a Community Mental Health Practitioner, you will play a vital role in providing high-quality care to adults with moderate to severe mental health difficulties.

Main Responsibilities

  • Hold a caseload and undertake the role of key worker for adults with moderate to severe mental health difficulties.
  • Offer support to junior staff and students.
  • Carry out the role of duty professional following a period of shadowing.
  • Support the delivery of group interventions.
  • Undertake initial assessments for new referrals into the service and provide support and cover to the established triage, assessment, and duty team.

Person Specification
