Job Title: Band 5/6 Mental Health Community Nurse

We are seeking a skilled and experienced Band 5/6 Mental Health Community Nurse to join our Older Adult Mental Health Team in East London.

Job Summary:

The successful candidate will be responsible for overseeing duty screening and urgent assessments, working closely with the team to provide high-quality care to older adults with mental health problems.

Key Responsibilities:
  • Conduct thorough assessments and develop effective care plans for older adults with mental health needs.
  • Collaborate with the multidisciplinary team to provide comprehensive care and support.
  • Develop and maintain strong relationships with patients, families, and carers.
  • Contribute to the development and implementation of care pathways and policies.
Requirements:
  • Experience of working with older people who have mental health problems.
  • Experience of working in a busy NHS setting, preferably in secondary mental health.
  • Understanding of dementia and its impact on the person and their wider support network.
