We are seeking an experienced and skilled Advanced Nurse Practitioner to join our team at Hallgarth. The successful candidate will be responsible for providing high-quality patient care, working collaboratively with the general practice team, and contributing to the delivery of best practice, policy, and procedures.
Main Responsibilities- Clinically examine and assess patient needs, plan clinical care, and implement treatment plans.
- Diagnose and manage both acute and chronic conditions, integrating drug- and non-drug-based treatment methods into a management plan.
- Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols.
- Work with patients to support compliance with and adherence to prescribed treatments.
- Prioritize health problems and intervene appropriately to assist the patient in complex, urgent, or emergency situations.
- Support patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self-care.
- Assess, identify, and refer patients presenting with mental health needs in accordance with NICE guidance for Mental Health.
- Promote and deliver evidence-based care for patients presenting with all conditions.
- Minimum 3 years post-registration experience.
- At least 2 years recent primary and/or community nursing experience.
- Nurse-led management of minor illness, minor ailments.
- Nurse-led triage.
- Leadership in quality initiatives such as clinical benchmarking.
- Advanced clinical practice skills.
- Management of patients with long-term conditions.
- Management of patients with complex needs.
- Clinical examination skills.
- Accountability of own role and other roles in a nurse-led service.
- Local and national health policy.
- Wider health economy awareness.
- Clinical governance issues in primary care.
- Independent non-medical prescribing qualification.
- Working with community development initiatives.
- Health-needs assessment.
- Compiling protocols and clinical guidelines.
- Relevant nursing/health MSc / Degree in Advanced Nursing Practice.
- Registered first-level nurse MSC or equivalent/Degree.
- Mentor/teaching qualification or equivalent experience.
