About the RoleThis is an exciting opportunity to work in a dynamic and supportive environment, where you will have the chance to make a real difference in the lives of our patients. As a Cardiology Clinician, you will work closely with our multidisciplinary team to deliver individualized care to patients, under the supervision of the District Nurse.
Key Responsibilities- Provide treatment, advice, and teaching within the professional competence of the post holder.
- Work as a member of the integrated community health care team, working within and across professional and organisational boundaries.
- Liaise with GPs, other professionals, and statutory and voluntary agencies to deliver individualized care to patients.
- Initiate patient assessment and care planning with the support of a senior colleague.
- Work with patients and carers as partners in care.
- NMC Registration.
- Ability to work as an effective member of a multi-disciplinary Primary Health Care Team.
- Ability to demonstrate knowledge of key issues in Primary Care Services.
- Excellent verbal and written communication skills.
- Car driver with valid UK Licence and access to a car for work purposes.
