Key Responsibilities- Provide holistic assessment and care planning for patients with complex and palliative care needs
- Develop and implement evidence-based care plans, working collaboratively with other healthcare professionals
- Deliver high-quality nursing care, including wound management, chronic disease management, and palliative care
- Work independently and as part of a team to ensure efficient and effective service delivery
- Communicate effectively with patients, carers, and other healthcare professionals to ensure seamless care coordination
- NMC Registered Nurse
- Experience of working in a community setting, preferably with a focus on palliative care and chronic disease management
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Good IT skills and ability to use electronic patient records
