About the Role

We are seeking a highly skilled and experienced Registered Nurse to join our Community Staff Nurse team at Powys Teaching Health Board. As a key member of our multi-disciplinary team, you will be responsible for delivering high-quality nursing care to patients in their own homes, promoting independence and improving health outcomes.

Key Responsibilities
  • Provide holistic assessment and care planning for patients with complex and palliative care needs
  • Develop and implement evidence-based care plans, working collaboratively with other healthcare professionals
  • Deliver high-quality nursing care, including wound management, chronic disease management, and palliative care
  • Work independently and as part of a team to ensure efficient and effective service delivery
  • Communicate effectively with patients, carers, and other healthcare professionals to ensure seamless care coordination
Requirements
  • NMC Registered Nurse
  • Experience of working in a community setting, preferably with a focus on palliative care and chronic disease management
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Good IT skills and ability to use electronic patient records
