Care Coordinator

We are seeking a skilled and compassionate Leaving Care Personal Advisor to join our team at Service Care Solutions. As a key member of our team, you will play a vital role in supporting young people aged 16-25 who are going through the asylum process.
Key Responsibilities:
- Provide guidance and support to young people to help them navigate the asylum process and achieve their goals.
- Work collaboratively with partners to identify and meet individual needs, ensuring that young people's plans reflect their strengths, aspirations, and needs.
- Deliver statutory responsibilities under the Leaving Care Act 2000, including conducting home visits, community work, and meetings.
Requirements:
- Experience delivering high-quality services to young people, families, or vulnerable individuals.
- Valid driving license and access to a suitable vehicle for travel.
- Experience in a similar role, with a strong understanding of the Leaving Care Act 2000.
What We Offer:
- A competitive hourly rate of 21-25 umbrella an hour, dependent on experience.
- A 37-hour working week, with opportunities for flexible working.
