Job Description

This exciting new opportunity involves working across Nottinghamshire as a Specialist Nurse, supporting healthcare professionals and patients in making informed decisions about treatment pathways in Diabetes. You will work closely with the NHS in primary care, delivering a unique nursing programme that drives best practice, excellent patient care, and improves patient outcomes.

Key Responsibilities

  • Deliver a defined geographical area, requiring excellent planning and organisational skills.
  • Work together with patients and practices to assist healthcare professionals in delivering up-to-date care for their patients.
  • Maintain intimate knowledge of national treatment guidelines and provide education, support, and advice to people with Type 2 Diabetes.

Required Skills & Qualifications

You will need:

  • A minimum of 3 years' experience (post-registration) managing diabetes within primary care.
  • Knowledgeable and confident in the provision of education, support, and advice to people with Type 2 Diabetes.
  • Skills and knowledge in the optimisation of oral hypoglycaemics.
  • IT literacy, including primary care systems.
