Vulnerable Pregnancy Support Specialist

We are seeking a Vulnerable Pregnancy Support Specialist to join our team at Hartlepool & Stockton Health GP Federation. The estimated salary for this position is £28,000 - £38,000 per annum, depending on experience.
In this role, you will work closely with the midwifery team to support vulnerable pregnant women and reduce health inequalities. Key responsibilities include providing one-to-one support to individuals and their families, helping people identify wider issues that impact their health and wellbeing, and working with commissioners and local partners to identify unmet needs within the community.
As a key member of our team, you will be responsible for introducing people to community groups, activities, and statutory services, encouraging people to volunteer and give their time freely to others, and tracking the impact of social prescribing on their health and wellbeing.
