We are looking for a skilled Clinical Advisor to support the delivery of high-quality person-centred healthcare to our patients in five care homes across Darley Dale.

The successful candidate will work autonomously, assessing, diagnosing, and managing acute and long-term conditions, with close collaboration with our GPs and multidisciplinary team.

Main duties will include:

  • Undertaking weekly visits to care homes to assess residents' health care needs and advance care plans.
  • Making referrals as necessary to other healthcare professionals.

This is an excellent opportunity to develop your skills in advanced care planning and experience working in a supportive team environment.

The salary for this role is approximately £53,000-£56,000 per annum, dependent on experience.
