Job Summary

We are seeking a highly motivated and experienced Case Officer to join our Short-Term Pathways Team in East Kent. As a key member of our team, you will work with adults over 18 with a wide range of needs, managing a small caseload and providing assessment, review, and arrangement of ongoing support to achieve improved outcomes and promote independence.

Key Responsibilities

  • Assess and review the needs of adults with complex needs, developing and implementing personalized support plans to promote independence and well-being.
  • Work collaboratively with health colleagues and other partnership agencies to ensure seamless care and support.
  • Provide ongoing support and guidance to individuals, families, and carers to empower them to make informed decisions about their care and support.
  • Contribute to the development and implementation of innovative solutions to address the needs of adults with complex needs.
  • Participate in regular team meetings and contribute to the development of team policies and procedures.

Requirements

  • Proven experience of working with adults with complex needs, preferably in a social care or health setting.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with individuals, families, and carers.
  • Ability to work independently and as part of a team, with a flexible approach to working hours and location.
  • Strong problem-solving and analytical skills, with the ability to think creatively and develop innovative solutions.
  • Knowledge of adult social care policies and procedures, with a commitment to promoting independence and well-being.
