Acute Care Nurse

We are seeking a highly motivated and experienced Acute Care Nurse to join our team at Lincolnshire Partnership NHS Foundation Trust. As a key member of our Crisis Team, you will play a vital role in providing high-quality care to individuals experiencing acute mental illness.
Key Responsibilities- Provide safe and effective assessment, treatment, and comprehensive discharge planning for individuals with mental health needs.
- Work flexibly across a 7-day-a-week service, including 9am to 10pm shifts, to meet the needs of our service users.
- Assess and identify the mental health needs of individuals, recognizing mental illness and developing interventions to meet their needs.
- Plan, implement, review, and improve interventions to manage risk and promote recovery.
- Collaborate with other healthcare professionals to develop effective care plans and promote good client care.
- Maintain accurate and timely clinical records, ensuring that all documentation is up-to-date and compliant with Trust policies.
- Registered Nurse (Mental Health) or equivalent qualification.
- Sound knowledge of clinical/risk assessment and understanding of Information Governance principles.
- Relevant experience of working with mental health needs, including assessment, treatment, and care planning.
- Highly motivated and able to engage with service users and carers to improve outcomes.
