Job Overview:

We are seeking an experienced Social Worker to join our Disabled Children's Team in Alton, covering North East Hampshire. The successful candidate will be passionate about providing quality services to children with disabilities and their families, delivered within a strength-based approach model.

Key Responsibilities:

  • Undertake holistic social work assessments to identify risk and need for children with disabilities.
  • Work closely with children and their families to develop and review plans that address and mitigate concerns in a timely manner.
  • Liaise with health and education professionals to develop support packages for children with complex needs.

Requirements:

A Social Work qualification is essential, and prior experience of working with children with disabilities is desirable. The ability to understand and navigate multi-agency systems and processes is essential.
