Job Title: Team Leader
Job Summary:
Elite is looking for a Team Leader to oversee care workers in the community and plan care packages. The ideal candidate will be passionate about delivering exceptional care and have experience in a care setting. As a Team Leader, you will be responsible for supervising and mentoring care workers, creating person-centred care plans, and auditing records. You will also be the face of Elite within the local community and participate in a backup/on call rota.

Main Responsibilities:
  • Supervise, mentor and lead the care team in delivering an exceptional standard of care to our service users.
  • Provide personalised support to care workers both in the field and offer guidance remotely.
  • Create person-centred care plans and risk assessments for people who use the service, using a computerised care planning system.
  • Review packages of care and adjust them according to the service users' needs.
  • Audit communication logs, medication charts and financial records.
  • Be the face of Elite Live-In Care within the local community.
  • Participate in a backup/on call rota.
  • Maintain good relationships with care workers, service users, stakeholders and the office team.

Requirements:
  • Experience in a care setting is essential.
  • Experience compiling care plans and risk assessments is preferred.
