About the Role:

We are seeking a skilled and compassionate Community Care Specialist to join our team at Bolton NHS Foundation Trust. As a key member of our community nursing service, you will provide high-quality care to patients in their own homes.

Job Responsibilities:

You will work closely with our experienced community nurses to deliver wound care, medications, end-of-life care, and continence care. A comprehensive induction program will support your transition into community working, ensuring you feel confident and prepared to meet the needs of our patients.

Requirements:

  • A Registered Nursing Associate on the NMC Register with a Foundation Degree
  • GCSEs Grade C or above in Maths and English language/New GCSE Grade 4-9 in Maths and English language OR Functional Skills level 2 in Maths and English
  • Experience of working in teams under appropriate supervision as part of a multidisciplinary team
  • Ability to organise and prioritise your delegated workload
  • Open-minded and respectful towards colleagues and the general public

Salary and Benefits:

The salary for this position is approximately £26,000 - £34,500 per annum, depending on experience.
