About the Role

We are seeking a dedicated and compassionate Social Worker to join our Adult Social Care Hospital Discharge Team (HDT). As a key member of our team, you will play a vital role in supporting individuals in different settings, enabling them to live independently.

Key Responsibilities
  • Provide assistance to individuals who require additional help to live independently, including those with disabilities or older adults.
  • Support individuals in their own homes, residential homes, short break services, or out in the community.
  • Collaborate with a range of stakeholders to ensure seamless care and support.
