About the Role

We are seeking a highly skilled and experienced Mental Health Practitioner to join our team at Lincolnshire Partnership NHS Foundation Trust. As a Band 6 AFC Specialist, you will play a crucial role in providing comprehensive Bio-Psychosocial Assessments and Evidence-Based Treatment Pathways for patients presenting to the acute hospital.

Key Responsibilities
  • Conduct comprehensive assessments and develop treatment plans for patients with mental health needs.
  • Provide operational support as part of the Mental Health Liaison Service (MHLS), working in partnership with United Lincolnshire Hospitals NHS Trust.
  • Work with individuals to assess their mental health, recognize mental illness, and identify their related needs and circumstances.
  • Plan, implement, review, and improve interventions to meet people's identified needs and manage their inherent risk.
  • Maintain accurate and timely clinical records and coordinate and monitor those of the team.
Person Specification
  • RMN, Social Work, or Occupational Therapist with professional registration (NMC) or degree equivalent to the profession.
  • Relevant experience of working with people with mental health needs, especially dementia and older adults.
  • Sufficient clinical knowledge to make autonomous decisions based on complex presenting problems and judgment about available options.
