About the Role

We are seeking a highly skilled and dedicated Staff Nurse or Therapist to join our Community Transfer of Care Hub. As a key member of our team, you will play a vital role in managing referrals into the Discharge to Assess Pathways across Dartford, Gravesend, and Swanley.

Working alongside a multidisciplinary team, you will be responsible for undertaking patient care needs assessments, visiting patients in their homes to conduct face-to-face assessments, and sourcing appropriate care provision to meet their needs. You will also be expected to communicate effectively with patients, carers, and partner organizations, maintain accurate patient records, and implement evidence-based care practices.

Key Responsibilities
  • Undertake patient care needs assessments to identify ongoing support required for safe acute hospital discharge
  • Visit patients in their homes to conduct face-to-face assessments of patient needs
  • Source appropriate care provision in the correct setting to meet patient needs
  • Ensure sourced care provision is implemented from the point of hospital discharge
  • Communicate effectively with patients, carers, and partner organizations
  • Maintain accurate patient records in line with professional and organizational requirements
  • Implement and work to ensure evidence-based care practices
  • Initiate health promotion activities with individuals, patients, and carers
  • Provide in-reach assistance to acute hospitals by providing expertise in community services to improve discharge and flow for pathways 1, 2, and 3
  • Undertake mentorship training and facilitate teaching and learning experiences for students and colleagues
