Job Overview:

The Inpatient Therapy Team at Chase Farm Hospital is seeking a Patient Care Facilitator to support patients with acute mental illness. As a dynamic team member, you will be responsible for delivering high-quality patient care and focused outcomes.

Key responsibilities include assessing, planning, implementing, and evaluating patient care under supervision. You must also have experience in group facilitation and be able to remain calm in stressful situations. A successful candidate should have an NVQ Level 3 in Care or equivalent training/experience and training in specific activities relevant to the client group.

This role offers an opportunity to collaborate with the local community and be involved in research and audit. The estimated salary for this position is approximately £25,000 - £32,000 per year.
