We are seeking a highly motivated and compassionate Reablement Support Worker to join our team at Northumberland County Council. As a Reablement Support Worker, you will play a vital role in supporting individuals to regain their independence and live their best life.

Key Responsibilities
  • Provide personalized support to individuals in their own homes, promoting independence and enabling them to carry out daily tasks.
  • Work collaboratively with individuals to set and achieve personal goals, improving their mobility, personal care, and overall well-being.
  • Support individuals with medication management, meal preparation, and other daily living tasks.
  • Develop and maintain strong relationships with individuals, their families, and other healthcare professionals to ensure comprehensive care.
  • Contribute to a multidisciplinary team, sharing knowledge and best practices to enhance the quality of care.
Requirements
  • Valid UK driving license and access to a car for work purposes.
  • NVQ2 or a commitment to achieving the Care Certificate Award and undertaking further training to meet the requirements of the post.
