Job Description

We are seeking a skilled Nursing Team Member to join our Integrated Community Nursing Care Team in Sheffield. The successful candidate will provide high-quality, patient-centred care to individuals in their own homes, working effectively as part of a multi-disciplinary team.

The role involves implementing packages of care and re-enablement programmes for patients, working under guidance and supervision from experienced nursing professionals. You will also be responsible for maintaining accurate records and reports of patient interactions and care plans.

This is an exciting opportunity to join a dynamic and supportive team that values its staff and the community it serves. We offer a competitive salary of £30,000 - £40,000 per annum, depending on experience, as well as opportunities for professional development and career progression within the Trust.

Main Responsibilities

* Provide high-quality, effective health care to patients in their own homes
* Implement packages of care and re-enablement programmes for patients
* Work closely with District Nurse Team Leader and Deputy to ensure seamless delivery of care
* Maintain accurate records and reports of patient interactions and care plans

Required Qualifications and Experience

* NVQ Level 3 in Health and Social Care or equivalent qualification
* Experience of working in a caring environment, preferably in a community setting
* Good communication and interpersonal skills, with the ability to work effectively as part of a team
* Ability to work on own initiative and unsupervised, with minimal supervision required
