Job Summary

Hatched Recruitment Group is seeking a highly skilled and compassionate Social Work Practitioner to join our team. As a key member of our team, you will be responsible for providing a professional social work service to adults, their families, and carers in Tower Hamlets.

About the Role

This is an exciting opportunity to work in a dynamic and supportive environment, where you will have the chance to make a real difference in the lives of our service users. As a Social Work Practitioner, you will be responsible for completing strength-based assessments and support plans, promoting independence, and ensuring that our service users receive the best possible care.

Key Responsibilities

  1. Develop and implement creative and personalized solutions to support individuals in managing their lives independently.
  2. Ensure that service users, including families and carers, are at the centre of decision-making over their support arrangements.
  3. Actively engage with, assist, and provide advice to carers, conducting carers assessments when appropriate.
  4. Promote independence by taking a preventative approach to interventions, maximizing the use of resources.
  5. Make ongoing assessments of need, identify and process safeguarding and quality of care issues, and refer on to the appropriate staff/agency.

Requirements

To be successful in this role, you will need to have a strong background in social work, excellent communication and interpersonal skills, and the ability to work in a fast-paced environment. You will also need to be able to work independently and as part of a team, and have a commitment to delivering high-quality services to our service users.
