About the Role:
As a Care Coordinator at Praxis Care, you will play a vital role in supporting individuals with mental ill health, autism, learning disabilities, and dementia. Your primary responsibility will be to lead a team of dedicated support workers, ensuring that our service users receive the highest level of care and support.
Key Responsibilities:
- Lead a team of support workers to deliver high-quality care and support services
- Develop and implement individual care plans to meet the unique needs of our service users
- Collaborate with other teams to ensure seamless delivery of services
- Monitor and evaluate the effectiveness of care plans and make recommendations for improvement
Requirements:
- Proven experience in a leadership role within a care setting
- Strong communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Knowledge of relevant legislation and regulations

£24,836 - £28,072 per annum (dependent on experience)
