About the Role

We are seeking a highly motivated and enthusiastic individual to join our Ferndown District Nursing team as a Community Healthcare Assistant.

The successful candidate will provide clinical care to patients in their own homes, under the supervision and support of the team. This role involves visiting patients in their own homes, providing evidence-based, high-quality, holistic nursing care, and following the instructions of the team leader or nominated deputy according to the needs of the patients as identified on the care plan.

Main Responsibilities

  • Undertake care following the instructions of the team leader or nominated deputy according to the needs of the patients as identified on the care plan.
  • Provide evidence-based, high-quality, holistic nursing care for patients in their own homes.
  • Work as part of a team to provide comprehensive care to patients in their own homes.

Requirements

  • Good communication skills and a passion for caring for patients in their own homes.
  • Experience of clinical work in the community is desirable but training will be offered to the successful applicant.
  • Access to a car with a valid driving license is essential for this role.
