We are seeking a skilled and compassionate Adult Social Care Worker to join our Mental Health and Wellbeing Integrated Care Team (MINT) in North Locality.
About the RoleAs an Adult Social Care Worker, you will play a vital role in providing person-centred support to individuals with mental health conditions. You will work collaboratively with a multidisciplinary team to engage with people, reduce support needs, and maximise independence.
Key Responsibilities- Complete Care Act 2014 assessments and draw up care plans with individuals, identifying their strengths and person-centred outcomes.
- Conduct Mental Capacity Assessments under the Mental Capacity Act 2005 and participate in Best Interest Decision making where necessary.
- Work alongside clinical staff to develop skills and knowledge in mental health, with ongoing support from a social care locality team.
- Access to regular reflective supervision and shared learning opportunities.
- A comprehensive Learning and Development programme, Quality Assurance Framework, and membership to Research in Practice (RIP).
- Hybrid working with flexible working hours policy and opportunities for career progression in Health and Social Care.
- Employee assist programme to support staff wellbeing.
This role requires a valid driving licence and own transport, or an alternative suitable method of travel. An enhanced Disclosure and Barring Service check is also necessary.
