About the Role


We are seeking a highly skilled and compassionate Strategic Community Care Professional to join our team. This exciting opportunity will allow you to utilize your expertise in supporting vulnerable adults, while fostering strong relationships with our community partners.



Key Responsibilities



  • Develop and maintain effective working relationships with healthcare professionals, social workers, and other stakeholders to ensure seamless support for our clients.
  • Collaborate with our team to deliver high-quality care services that meet the unique needs of each individual.
  • Cultivate strong communication skills to facilitate successful partnerships with clients, families, and external agencies.
  • Demonstrate commercial awareness by identifying opportunities for growth and improvement within our services.
  • Prioritize business growth targets by implementing strategies that enhance client satisfaction and retention.
  • Build strong organizational and planning skills to effectively manage multiple projects and priorities.
  • Maintain flexibility to adapt to changing demands and requirements within the organization.


Required Skills and Qualifications


To be successful in this role, you will need:



  • A valid driving license and access to a vehicle to facilitate travel throughout the community.
  • Excellent customer service and man-management skills to work effectively with clients, colleagues, and external partners.
  • Strong influencing skills to build relationships and negotiate with stakeholders.
  • The ability to demonstrate achievement of business growth targets through proactive planning and execution.
  • Excellent organization and planning skills to manage multiple tasks and responsibilities.
