About the Role

We are seeking a highly skilled and motivated Occupational Therapist Specialist to join our team at Central and North West London NHS Foundation Trust. As an Occupational Therapist Specialist, you will play a vital role in providing high-quality care to our patients, working collaboratively with our multidisciplinary team to deliver person-centered rehabilitation and individualized goals.

Key Responsibilities

  • Provide comprehensive assessments and deliver person-centered rehabilitation and individualized goals.
  • Undertake all aspects of clinical duties as an autonomous practitioner.
  • Work in the Community pathways, assisting in admission avoidance assessments, the Getting People Home pathway to facilitate early supported discharge from the acute hospital and Inpatient rehabilitation pathways.
  • Communicate effectively and work collaboratively with all members of the multidisciplinary team and other services within the Trust, as well as outside agencies.
  • Ability to work in capacity as a Trusted Assessor.
  • Participate in development of integrated pathways.

Person Specification

  • Ability to drive and has access to a car for use at work.
  • HCPC registration.
  • Degree level qualification in Occupational Therapy.
  • Experience post registration in community / acute / stroke / rehabilitation settings.
  • Previous clinical experience of working in an NHS setting.
  • Evidence of recent continuing professional development.
  • Experience post registration in stroke / rehabilitation settings.
