Community Physiotherapy Specialist

We are seeking a qualified Physiotherapist to join our team in Bristol, providing reablement services to patients in their own homes.

Job Overview:

The successful candidate will work closely with our team to deliver high-quality care and support to individuals, helping them to maintain independence and prevent hospital admission.

Key Responsibilities:

  • Conduct comprehensive assessments of patients' needs and develop personalized care plans
  • Deliver physiotherapy interventions in patients' own homes, focusing on prevention of hospital admission and support of discharge assessments
  • Work collaboratively with multidisciplinary teams to ensure seamless care delivery
  • Document patient progress and outcomes accurately

Requirements:

  1. Registered Physiotherapist with a UK driver's license and access to a car for work purposes
  2. Experience working in community settings or similar environments is essential
  3. Strong communication and interpersonal skills, with the ability to build rapport with patients and colleagues alike

Salary Estimate: The salary for this role is estimated to be around £35,000 - £45,000 per annum, depending on experience.
