Senior Care Leaver Support Specialist

Job Summary
The Leaving Care Social Worker is responsible for supporting young people aged 16-25 as they transition from the care system to independent living. This role involves working closely with care leavers, helping them navigate challenges in housing, education, employment, and mental health, while ensuring they have the necessary life skills and support networks to thrive in adulthood.
Key Responsibilities
- Develop and implement personalized support plans for care leavers to address their unique needs and goals.
- Collaborate with local authorities, schools, and community organizations to provide comprehensive support services.
- Conduct regular check-ins and assessments to monitor care leavers' progress and provide guidance and resources as needed.
- Facilitate connections with employment, education, and training opportunities to help care leavers achieve their career aspirations.
- Provide emotional support and guidance to care leavers as they navigate the challenges of independent living.
Requirements
