This exciting opportunity with the NHS will see you working as part of the Heart Failure nursing team, providing specialist assessment, planning, and treatment to patients suffering from heart failure. You will be visiting patients in their homes and within the hospital setting, using advanced clinical knowledge and skills to deliver high standards of patient care.

Main Responsibilities
  • Conduct specialist assessments to identify patients' nursing needs and develop individual care plans.
  • Collaborate with the Heart Failure Team to contribute to the National Heart Failure Audit.
Key Responsibilities

As a Heart Failure Nursing Professional, you will be responsible for:

  • Delivering high-quality care to patients with heart failure, including assessment, planning, and implementation of care.
  • Contributing to a multi-professional approach in the management of patients, particularly in relation to ongoing care needs, admission avoidance, and discharge arrangements.
  • Providing support and education to patients and their families, enabling them to self-manage their care and take an active role in their treatment.
  • Liaising with palliative care teams regarding patients with end-stage heart failure.
  • Supporting the nurse-led outpatient clinic for patients requiring ongoing care assessment and counseling services.
Requirements

To be successful in this role, you will need:

  • Recent experience of managing and caring for cardiac patients, including heart failure.
  • Outpatient clinic experience.
  • Knowledge of health promotion principles and practices.
