About the Role

The Discharge Coordinator will play a crucial role in ensuring the smooth discharge of patients from Ward 31 at Darlington Memorial Hospital. This busy Trauma and Orthopaedic Unit cares for patients with trauma injuries and is also the designated area for patients admitted with head injuries.

The successful candidate will work closely with the nursing team to ensure that all patient discharges are maintained to a high standard. This will involve liaising with other parts of the Multi-Disciplinary Team (MDT), including doctors, nurses, OT, and physio, to ensure that complex needs have been reviewed and a safe hospital discharge is achieved.

Main Responsibilities

  • Support nursing staff with all discharges, including complex ones
  • Liaise with MDT members to ensure a safe hospital discharge
  • Build good relationships with patients and their families to facilitate a smooth discharge
  • Communicate effectively with patients to support their discharge home
  • Support nursing staff with complex and difficult discussions around discharging concerns

About Us

County Durham & Darlington NHS Foundation Trust provides hospital services from two acute sites, Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield, and Weardale, as well as over 80 other community-based settings and providing care in patients' homes.
