About the Role:
- To provide expert clinical guidance and support to service users, empowering them to achieve optimal health outcomes.
- To conduct thorough initial assessments, develop tailored care plans, and review ongoing care plans and risk assessments in collaboration with patients, families, and healthcare professionals.
- To maintain accurate and up-to-date clinical records, including Medication Administration Records (MARs).
- To observe care staff, assess their competence, and provide general clinical oversight of the branch and service users.
Main Responsibilities:
- Assessing and implementing care plans for individual service users.
- Conducting medication audits – monthly, via Birdie.
- Providing direct care to service users to maintain hands-on clinical skills.
- Building strong relationships with parents, their families, and carers.
- Collaborating with multidisciplinary teams.
- Maintaining accurate administrative records.
Salary and Benefits:
- A competitive salary of £45,000-£55,000 per annum.
