About the Role:

  • To provide expert clinical guidance and support to service users, empowering them to achieve optimal health outcomes.
  • To conduct thorough initial assessments, develop tailored care plans, and review ongoing care plans and risk assessments in collaboration with patients, families, and healthcare professionals.
  • To maintain accurate and up-to-date clinical records, including Medication Administration Records (MARs).
  • To observe care staff, assess their competence, and provide general clinical oversight of the branch and service users.

Main Responsibilities:

  • Assessing and implementing care plans for individual service users.
  • Conducting medication audits – monthly, via Birdie.
  • Providing direct care to service users to maintain hands-on clinical skills.
  • Building strong relationships with parents, their families, and carers.
  • Collaborating with multidisciplinary teams.
  • Maintaining accurate administrative records.

Salary and Benefits:

  • A competitive salary of £45,000-£55,000 per annum.
